New Patient Registration

  • Come along to reception and complete a new registration form, please bring proof of Identification.  Please note your Registration is not complete until you have produced identification and signed the form at the Health Centre.
  • Complete a health questionnaire (this is helpful until your previous records become available). 
  • Make a new patient appointment with the practice nurse for a general health check.

The forms can also be downloaded from the above Registration Forms tab.  Please note your Registration is not complete until you have produced satisfactory identification and signed the form at the Health Centre.

Practice Area

Patients will be accepted on to the doctors’ list from Glen Isla, Prosen, Uig, Clova, Moy, Quiech, Ogil and Ogilvy and from Airlie, Ruthven, Eassie, Glamis, Padanaram, Tannadice and Fern and all areas within these limits.

District nurses based at the health centre cover much of this area.
